IV Phosphatidylcholine Protocols

Indications

Hyperlipoprotein (a); hypertriglyceridemia; elevated LDL cholesterol
Cancer
Neurodegenerative diseases (ALS, MS, Parkinson’s disease, Dementia)
Atherosclerosis and arteriosclerosis
Chronic Fatigue states
Liver disease
Angina Pectoris and peripheral vascular disease
Nephrotic Syndrome
Fatty embolism

Contraindications

Allergy to PC
Antiphospholipid syndrome
Infants due to the alcohol content

Side-effects

Diarrhea
Fatigue (temporary)
Temporary elevation of HDL, LDL and liver enzymes
Phlebitis or thrombophlebitis if given to quickly

To reduce side effects and the incidence of thrombophlebitis, increase the infusion time to 2 hours, and when given it IV use 500 cc instead of 250 cc of D5W (Only use D5W)

Use Becton Dickinson (BD) or Braun Teflon catheters
Do not use Terumo catheters because they interact with the PC.

You can use a 22 to 25-gauge Butterfly needle.

Do not give the IV too quickly, hypotension can happen.

Asians rarely can be sensitive to PC and develop temporary fatigue when PC is given too quickly

Precautions

Follow the protocol exactly to prevent thrombophlebitis
Use only the pure phosphatidyl choline

Recommended lab test

Fasting lipid panel
Homocysteine, CRP-HS, Fibrinogen
CBC
Urinalysis and microalbuminuria
Complete Chemistry Panel including GGT and free calcium
EKG, Carotid Doppler ultrasound if being used for atherosclerosis
64 slice cardiac CT if being used for atherosclerosis


IV Push Protocol (Not too affective for atherosclerosis)

Use a 10 cc syringe, put 0.5 cc of heparin (5,000 units/ml) in the syringe; use a 22-25 gauge Butterfly; add 5 cc (250 mg) (1 vial) of the phosphatidyl choline to the syringe, gently draw out 5 cc of venous blood, gently rock the syringe to mix the blood and the PC together, and then infuse back into the body for 15 minutes.

This can be given twice a week.  Elevated CPK may decrease.

Long IV for atherosclerosis

2-3 treatments per week (serum half-life is 32 hours)
48 hours between treatments
30 treatments minimum for atherosclerosis
Maintenance therapy: 1-2 treatments a month.
Store PC in the refrigerator.  Do not reuse. Shelf life if not opened is usually 6 months.  Before the IV, make sure the PC is at room temperature.

Only mix the PC in D5W, nothing else!
Start with 20 ml (1,000 mg) of PC in 500 ml of D5W
2 days later increase the dose to 30 ml (1,500 mg)
2 days later on the 3rd IV and subsequent IV’s use 50 ml (2,500 mg)

The length of infusion is 120 minutes.

Patient who are Asian or who weigh less than 120 pounds (55 kg) should only have 40 ml (2,000 mg) maximum per IV

If doing EDTA chelation the same day, do the EDTA chelation and PC IV separately. Usually do 1 EDTA per 1 PC IV.  In severe heavy metal toxicity, the ratio can be 2 EDTA’s per 1 PC IV
